Hiring debrief
A hiring debrief is the meeting where everyone who interviewed a candidate shares their assessment and reaches a decision. Run well it takes twenty minutes; run badly it becomes an unstructured discussion dominated by whoever is most senior or most recent.
The single most effective change is having interviewers submit their assessment before the meeting. Once someone has heard a colleague's opinion, their own recollection reshapes itself around it — this is anchoring, and it is powerful enough to make a debrief little more than a consensus around the first person to speak.
The second is going in a deliberate order, often most junior interviewer first, so seniority does not set the anchor.
